Physical Therapy Assistant

We are seeking a Licensed Physical Therapy Assistant to join our team working 32-40 hours. In this role, you will support patient rehabilitation and recovery efforts, contributing to our mission of delivering exceptional healthcare services. This position is vital for ensuring our residents receive personalized and effective therapy tailored to their needs.

Responsibilities

  • Assist licensed physical therapists in implementing treatment plans and therapies.
  • Monitor patient progress and document responses to treatment.
  • Provide therapeutic exercises and interventions tailored to individual patient needs.
  • Educate patients and families about post-treatment care and exercises.
  • Maintain accurate records of therapy sessions and patient progress.
  • Ensure a safe and clean environment for therapy activities.
  • Collaborate with healthcare team members to optimize patient outcomes.
  • Support discharge planning by preparing patients for post-therapy activities.

Qualifications

  • Valid MA state licensure as a Physical Therapy Assistant.
  • Strong knowledge of physiotherapy techniques and patient care practices.
  • Excellent communication skills with patients and healthcare team members.
  • Ability to work effectively in a fast-paced environment.
  • Knowledge of medical terminology, anatomy, and physiology.
  • Compassionate attitude with a focus on patient well-being.
  • Prior experience with geriatrics is highly desirable.

Certified Nursing Assistant 

We are seeking CNA’s to join our staff in a per diem capacity.

Qualifications:

  • High school diploma or equivalent (GED)
  • Certified Nursing Assistant Certification
  • Dedication and passion to serve seniors with excellent customer service skills
  • Must be at least 18 years of age
  • Previous experience working with seniors preferred

Director of Nursing Services

This role is responsible for planning, organizing, and directing all nursing operations to ensure high‑quality resident care and full regulatory compliance.

Primary Responsibilities

Leadership & Department Oversight

  • Direct the daily operations of the Nursing Department and ensure consistent delivery of quality resident care.
  • Develop, maintain, and update nursing policies and procedures.
  • Determine staffing needs and partner with HR and the Executive Director on recruitment and retention.
  • Participate in facility committees including Care Planning, Discharge Planning, Compliance, and Safety.

Regulatory Compliance & Quality Assurance

  • Participate in state and federal surveys.
  • Develop and implement plans of correction for identified deficiencies.
  • Ensure adherence to all infection control standards and safety protocols.
  • Maintain knowledge of all applicable nursing regulations, guidelines, and best practices.

Staff Development & Performance Management

  • Collaborate with the Staff Development Coordinator to plan and coordinate annual education and all in‑service as needed.
  • Manage performance expectations for nursing staff, including annual evaluations and corrective action processes.
  • Support nurse managers in coaching, development, and team leadership.

Resident & Family Relations

  • Address and resolve resident and family concerns or grievances in a timely and professional manner.
  • Promote a culture of communication, respect, and resident‑centered care.

Requirements

  • Valid Massachusetts RN license in good standing.
  • Minimum of three years of supervisory experience in a hospital or skilled nursing facility.
  • Prior DNS or ADNS experience required.
  • Demonstrated ability to lead, coach and manage nursing staff effectively.
  • Strong communication and organizational skills.
  • Competence with basic Microsoft office skills.
  • Experience with electronic health record, preferably PointClickCare (PCC).
  • Ability to work tactfully and professionally with staff at all levels, residents, families, medical personnel, visitors, vendors, and regulatory personnel.
